Age Friendly Heathcheck Tool

We want an online tool that will allow users to answer 16 questions - Yes, No, Don't Know. The responses will produce a customised downloadable PDF report. The report will have an introduction (fixed text) and 6 sections. Each section will comprise 3 paragraphs. Para 1 fixed text. Para 2 algorithm generated text, para 3 a set of recommendations. Each section will have a traffic light indicator. Need a sign up page which collects basic data and has payment gateway, questionnaire which is 1 page and general info page. Responses will be stored in a database. Require architecture, build and graphics overlay. We will provide text and linkages. Examples of 2 similar tools in attached RFP
