Create 5 forms for my website; the submit button must links to paypal to collect payment

My website has 5 'products'. Rather than using a shopping cart/e-commerce solution, I need each product to have a page that is a form where certain information is entered by the customer. The submit button of the form needs to be a direct link to my paypal merchant account where the customer pays for the item.
One of these product pages needs to embed a map of the UK and lets the customer draw an outline on the map to identify a piece of land or a building. Their selected area needs to be submitted to me along with the form.
I will supply all the form fields, prices etc and show you a similar form to what I am looking for.
You will need to do this within my existing site and theme, which is a wordpress site using the Avada theme.
You will need to be able to do this in a way that is fully editable by myself within wordpress when you are finished - nothing is to be hosted on other sites that I can't control.
