Create Interactive & Printable PDF form

Design interactive and printable pdf for our internal use. We use the 1-page pdfs for QA tests and record keeping. Interactive ares will be: 1. Auto prefill date (with manual entry if needed) 2. Checkboxes 3. Numeric entry boxes 4. Pull-downs 5. Prefill with default states we set up (checkboxes, fields, signature, etc) 6. We would like to use a USB barcode reader to scan in a serial number which will populate a designated field in the pdf and display the barcode. 7. Two of the user actions will cause images to appear in the document that correspond to the physical item being tested. Please find the attachment for the items we will have in the PDF you create. We may add a few more at the start of the project but this is the bulk of the items. For the barcode serial number field please provide us with the requirements for the physical reader that will integrate into your form. We have not purchased the scanner yet. We have Adobe Acrobat Pro so you can set up to allow us to add items and change defaults. Other than that we just want a good design and clean layout with our letterhead, watermark and auto-fill images. We are ready to begin immediately.
