Create PDF form for new account creation requests

The key functionality request of your project is file attachment. There are few options which I can offer, with certain restrictions. 1) You can have Add Attachement button programmed, but it will only work for Acrobat Standard or Pro users, so this option would only make sense if you know that all of your form users have that software. This works when you design the company intranet form, but not the best option if you want to share your form to unlimited user group. 2) Modern dynamic pdf form can also have the Image upload field, where user can add .jpg, .png or .tif scans. However, dynamic pdf forms are only 100% compatible with Adobe Reader software, which is free and available for mac and pc computers. Dynamic pdf form will not work with 3rd party pdf readers. 3) Another option would be to add a Submit by email button, which will just open the user email client and generate a message with the pdf file attached to it. Then user can attach all kinds of files to this email message, like you would normally do it with your email.
