basic web form coding

I need attached "contact us" page coded so the results entered are e-mailed and then user is re-directed to home page. Thank you page possibly??? I will provide email address to whomever is awarded the project. I need the "Display Field" on the results email to read "[login to view URL](dot)extractsoftware(dot)com" when the forms are submitted. There is no database involved. All fields required except comments.
