Create textbox on an image and send to MySQL

Hi Folks, I just create a website (with host and domain name) and want to seek for long-term collaboration with a developer/wordpress specialist (up to a year). I will have some tasks available per week or per 2 weeks in first 3 months, and maybe 1 per month thereafter. If the initial task result is satisfactory, i am glad to retain the relationship with 1 to 2 developers. This week I need to get an image (already designed) on a webpage (with a couple of blank area on the image to receive user input) filled with textbox, checkbox, forms, CAPTCHA,submit box. Some HTML form knowledge (or with the help of wordpress plugin) is required. - This will be 0.5 to 1.5 hour work. After user input and "submit", it will pass those values to MySQL. Some MySQL tables need to be inserted/created. A php file may be necessary. - This will be 1 to 2 hour work. Here is a bit challenge part: There is a checkbox (optional to select) in above design to "agree to publish user's comment". If it is ticked, user's name and comment will be shown in the same image but locate in exact location. - It is better that website administrator can approve or decide to show the comment or not - It is recommended to show 9 user's name and comments in total in the image (some blank area for them), and if a new user submit a comment with checkbox ticked, 1 old name/comment will be automatically replaced by the new one. Detailed illustration will be provided for sure before and during design.
