Day-by-Day Itinerary online form (connected to wordpress site)

The company I am employed by is in need of an online solution that will allow clients to log into our website and fill out a form listing all bus transfer services that they need for us to provide (we are a bus transportation company). The form would entail a set of fields to enter information concerning the service (date, time, from, to, observation).

However, the tricky part that has led me here (because I could not figure out a way to do it on my own so far) is to have a dynamic table that allows the user to enter multiple services before they submit them to us. There would essentially be an "Add to Services" button that would add the new service to a dynamically growing list of services that they will submit altogether. There would also be some conditional logic involved depending on user input, and each field would need to be formatted to suit the type of data being entered (ex. date format for date field, time for time field).

Another important aspect of this project would be that the clients would need to have their own account through a login/password where they could submit their itineraries and also see a list of their previously submitted itineraries. Our website is currently set up through WordPress, so if it is possible, the user registration could be run through the actual site, and after logging in they would be redirected to a page showing the option to submit a new itinerary or view a previous one.

On our end, we would need to be able to retrieve the information submitted by the clients through excel. Exporting to CSV is an option, but I would much prefer the export as a .xls file. Each service would need to have its own record in the spreadsheet, also containing a header at the top of the spreadsheet with additional information about the client/submission. We would also need to be notified via email when a new submission has been made with all its appropriate information and with the excel file as an attachment.

In the future, we would like to be able to connect all this information to a database where we could automatically retrieve submissions and have them added to an overall inventory that could be sorted and analyzed.

Sidenote: I am aware that many of these features are already available through existing form plug-ins for WordPress (ex. Gravity Forms, Formidable), but they all lack the key feature of having a data grid to allow multiple services to be entered. I have done some searching and have seen that a possible solution to this problem might be able to be implemented through the use of jQuery datagrids, but I am not sure how this solution would be put into effect. This is merely what I have been able to find, but I am sure there are much more suitable options for what I am looking for.

We are in need of this solution to be implemented on our website promptly, so I would need the completed project in the near future. Thanks in advance for any offers and I hope to begin working on the project soon!
