Commission Linkage

I am writing a book in directory form and need an Affiliate Linking program/code to sites without an affiliate program. The recipiant would be an email address different for each page/company. If I introduce a client who buys I get paid a commission. It is my intention to sell the book in CD ROM format. One page per company. This page would be filled in by the book buyer, with all fields needing completion (except Mobile phone), on sending, this page would go to a pre-designated email address. NOTE: The company would manually transfer the buyers details onto their own database on receipt. Requirement. A code? with company number 1 details keyed into the code. I would cut and paste this into the page set in Microsoft Word 2000. The program to contain the following page details First Name Surname Address line 1 Address line 2 Town/City County Post Code Daytime phone Mobile phone Email address retype email I would then change the code to reflect the details of company number 2. This will be the email address for it to be sent to. The initial link would best be a message, say "Click Here to Register". This would take the book buyer to the registration page.
