Add form fields/buttons to existing web page

What I need is to have a multi-tab (or multipart) form graphic inserted into the body of a web page. The form graphic has already been created in InDesign, and now it needs to be imported into the page body.? The fields and? buttons in the graphic representation of the form need to be replaced by actual fields and buttons on the web page. This project is for a client who has a coder who handles his backend stuff, so nothing needs to be tied it. Their coder will add the proper code to the buttons, etc. For your assistance, I have here a flowchart of the "form" graphic in its different states, as well as a pdf of the web page itself as it will look with the form inserted. It will be placed under the header and menu bar (will not be changed), and it will be replacing the content that you currently see in this area. The advertisements on the right will remain. Upon acceptance of a bid, I will send you a zip of the InDesign document as well as either the ftp info for the site, or the home page file for you to work on. This should be an easy job for an experienced coder. I'll be getting up at around 6 am here to look over bids and accept one. Please ONLY bid on this job if: 1. you are able to start as soon as your bid is accepted, which will be by 6 am eastern US time. 2. you are able to work on this throughout the day tomorrow; and, you are able to get this job done in a timely manner--I've quoted 10 hours on the work itself, and believe that it should not take longer. Therefore, it would be appropriate to have the form integrated into the page by Thursday night / Friday morning. Please do not bid if you aren't comfortable with this deadline. 3. Cherie ## Deliverables Please be mindful that the actual completed form needs to closely resemble the form graphic created in InDesign. I could do this job myself, but I hate creating form fields, so I need the assistance of a coder! Also, in the example instructions pdf, the form is depicted as having functionality of making green and/or red checkmarks appear as the user completes each form field. Not so!! This is not to happen--bad idea which was ditched, just please make it nice and tidy with none of that. Also note that at least one of the tabs at the top of the form will basically not be done right now, and will simply say something like "Coming Summer 2008 (you will see this on the flowchart)."
