checkboxes for user permissions

I need a script for a simple grid/table with checkboxes linked to mysql database. The script would get list of data (i.e. pages) from database which would be displayed in cells on top row of the table and list of users which would be displayed in cells on the left side. All the other cells should contain checkboxes and if user has permission to acces one of the pages the box under it should be checked. If box is selected or deselected and then saved permissions for users would be updated in database.
