CSV to HTML script development

I am very interested in seeing whether a variation of the CSVRead script from [url removed, login to view] would be the right solution for what I am trying to achieve. I have a widely distributed Palm application that users use to log particular events. Each of the entries contains about 40 pieces of information (name, number, date, etc). I am able to export the data from the Palm to a CSV formatted file.

Please note that each user will have their own CSV file. CSV files are NOT shared between users. The web site is simply allowing the users to view the their own CSV data in a nicely presented way.

I would like to have a couple of server stored web pages that would permit the following:

1) Read the CSV data from a file stored on the users local computer (user defines the file location with a text field and "Browse" button)

2) First page shows a list of all the entries (linked) in the CSV. The list would show 4 specific fields for each record.

3) Clicking on an individual entry link above would take the user to another page that would show all the data for each entry. The data would have to go into very specific table cells in the html document. The graphic layout will be very specific; I will design and produce the html code for this.

Additionally I would like the entry list (item 2 above) to have a specific number of maximum records, once the records are exceeded, there would be a "crumb trail" of pages shown. Similar to the sample here:

[url removed, login to view]

Additionally, at first, the user should be able to select with type of measurement units to use; either metric (meters, Celsius, kilograms) or imperial (feet, Fahrenheit, pounds) and certain text labels on the web pages change depending on the selection made. Also, some of the fields in each record are "True/False" entries, which I would like to show up as checkboxes on the web page.

I would need someone to create/modify the CGI scripts, and upload to my host server. My host supports CGI scripts. I have almost no experience implementing CGI scripts. I need help in setting up the templates, so I can design a user friendly, professional and attractive interface.

I need to get an idea of the cost and timeframe involved.
