C# Win Form Treatment Plan

A C# Win Form Application (sub module) is needed that contains only one Form, the form contains one image of teeth that dynamically adapt the changes added to each tooth. The change could be any kind of the treatments which are provided. each tooth can take any number of treatments(based on the requirements) or it can be disabled. the added treatments should be saved in a table in SQL database, changes should be done fast. there is a kind of treatment that combined more than one tooth. some treatments need a selection area, we will provide all the needed information, the database design is ready. also we will provide the contractor with a sample application which demonstrate the needed requirements. beside the image a list should show all the added treatments to each tooth. all controls should be re-sizable. also all controls should be put on panels not directly in the form. the form should be implemented in Visual studio 2010 and Microsoft SQL 2008. the code should be well documented.
