Create a mobile service scheduling website / app

~~~About 9 pages total when the site is done~~~ I would like to have an application created for a service industry. The application will be very basic as far as functionality. A series of buttons that take you to different steps to complete a scheduled service. For example: If the service was scheduling a hair cut, or scheduling an appointment for tire rotation, you would go to the application, schedule the appointment, and you can pay in person, or in the application. The scheduling part would need to work and pass testing of course. There are about 5-6 services at different prices. I would like the user to check the boxes for what they would like, that will total the price like a shopping cart would, and they can pay in person, or in the application. Once the order is placed, I would like to be able to see all orders from the admin side of the application. This can be an application, or a mobile website. If you can make a mobile websaite into an easy application, then that would work as well. I would like full rights to the software or website, and I can host it online myself. I just need assistance building it. I am looking at a small budget for getting it working, so graphics do not need to be amazing. It needs to function well, and look decent, and I will pay more money later as we continue to develope it. I have sketched out what the pages will generally look like. I attached some examples of the application. I just need the application or mobile website to flow. These are rough sketches, and I would like to be able to change the wording on everything if possible.
