Pride Vocational Services is a very small company that does vocational rehabilitation services for workers' compensation cases. I need to have a database designed and program written that allows me to track cases and automatically generate and send a PDF of the state-mandated billing form. I also need it to track bills that are sent and when payment is received. There are many other business processes that I would like to eventually automate/improve with software, so a good experience would likely lead to requests for phased expansion of the scope of the software in the future to include: - Generation of additional forms and letters - expansion to support cases in other fields closely related to vocational rehabilitation - Support for online use of the software to allow a secretary to enter case information remotely ## Deliverables Software should allow me to: - enter new case information - enter information about MCOs (the organizations that assign cases to me and pay me) - enter details of work that I do on each case - allow me to select a case and have a billing report generated (report format must very closely match the format of the state-mandated billing form) - generate a PDF of the completed billing form and email or fax it to the appropriate MCO based on MCO preference (if integrated faxing is a big problem, then print the form so I can fax it manually) - track which line items were billed and mark them as paid when a check is recieved - allow for partial payment of a bill and for re-billing of line items that were rejected Attached files show some samples of the current Excel sheet we use to handle biling. There is one set of blank forms, one set of forms that was filled out for one of our cases, and one page of the tracking sheet to keep track of what bills were sent and what bills have been paid. NOTE: the main form (C19) that this software needs to generate is included in the "blank billing [login to view URL]" file with lots of notes about where each piece of data comes from. Hopefully this will be very helpful to understand the requirements. Current C-19 form: [login to view URL] Current fee schedule: [login to view URL] Database: MCO - details of each MCO organization (the companies that assign cases to us) MCO ID, Name, Address, Telephone, Email, Fax, PreferredBillingMethod(Email or Fax), ContactName, comission percent, etc PROVIDER - details of each vocational rehabilitation counselor Provider ID, Provider Number, Group Payee Number, Name, Address, Telephone, Email, Fax, default (mark this as the default provider for all new cases) IW - details of each injured worker IW ID, Name, Address, Telephone, Email, Fax, Social Security Number, etc. CASE - details of a case assigned to us CASE ID, IW ID, MCO ID, PROVIDER ID, Claim Number, Date Of Injury, ICD9 Codes (may be several codes), EmployerName, Employer Contact Name, Employer Email, Employer Phone, Employer Fax, Date Of Referral BILLING CODE - Ohio BWC Billing Info (A.K.A Procedure Code) These are set by the state and change from time to time. Need a way to import/update these. Current Fee Schedule is available from [login to view URL] CODE ID, Code, Description, Unit of Service, Fee Description, Fee (dollar amount to bill) CASE NOTE - work history for cases - this is the basis for the C-19 NOTE ID, CASE ID, PROVIDER ID (usually, but not always == [login to view URL] ID), CODE ID, Date, Units, Notes, C19 - Info for a specific bill (invoice) sent C19 ID, CASE ID, Start Date, End Date, Comments (i.e. this is a rebilling of previously denied charges), Total Billed, Amount Payable (total billed - MCO comission), Amount Recieved, Recieved Date CASE REPORT - like a C-19, but an informal report of work done. Includes billable and non-billable case notes REPORT ID, CASE ID, Start Date, End Date, Comments ACTION ITEM - a report, follow up action, etc for a case. ACTION ID, CASE ID, Description, Due Date, Completed C19-CASE NOTE - list of case notes included on a specific C-19. Only notes with a billable billing code are included C19 ID, NOTE ID, RebillFlag (sometimes charges are denied and they need to be modified and rebilled), IgnoreFlag (sometimes charges that are denied should never be rebilled, they are just invalid) REPORT-CASE NOTE - list of case notes included on a specific case report. All case notes (billable and non-billable) are included. No "rebilling" or reconciliation process needed for these. REPORT ID, NOTE ID Need a way to run billing for a specific case - specify a date range and generate a C-19 for that case for the specified date range and **preview** the bill and email or fax it (depending on [login to view URL]) and also save a PDF of the file to the local computer. Need a way to run rebilling for a specific case note (or multiple notes). Search by case by date range. Include all C19-CASE NOTES flagged for rebilling that are not also flagged as ignore. Need a form to update each of the sets of information for each entity (MCO, PROVIDER, IW, CASE, BILLING CODE, CASE NOTE, C19, CASE REPORT) Need a form to create a new case. Need a form to show a summary of current open cases, upcoming deadlines for each case, and outstanding invoices for each case. This should be the first screen displayed when the software is started. (see page 1 of software design sketch) Need a form to enter case notes - this is a form that's used every day to record work as it is done. (see page 2 of software design sketch) Need some basic reporting to show amounts billed, recieved, outstanding, etc. overall and per provider per time period Need a report for tax purposes that shows all travel expenses for a specified calendar year. It would be nice to be able to select which billing codes show up in this report. For example, we will probably create a custom billing code for non-billable mileage that would never get included in a C19, but is still deductible as a business expense for tax purposes. (see page 2 of software design sketch) The sample completed billing form that I posted contained only 1 page, but sometimes they can be 2 or more pages. The preferred way to handle multiple pages is to insert a line break and push the extra rows (and the footer data - signature, etc.) down to page 2, inserting a line at the end of the previous page that says "continued on page 2". The new page should have a header stating the claim number, IW's name, and "page # of #. This is the way to handle up to 75 billing lines on a single form. If there are more than 75 lines, then a 2nd C19 form should be generated since the state will only accept up to 75 rows on a single form.
