Cordova/Jquery app to create word doc from database

Hello all, I have created an iPhone application with cordova (phonegap) and jquery mobile which allows people to take photos and add notes to the pictures. All of the information is stored in a simple 2 table database. I now require that this information be able to be outputted to a microsoft word document (preferrably a word XML document). I have attached the report template to clarify what i mean. Each row will get it's data from a row in my database, a second table will be created with the photographs and aditional information. As my iPhone application is written in Cordova and Jquery, this code will need to be written in similar. This is only a simple project, I have succeeded in creating my own solution by copying the XML from my report template in to my code and looping through my records, however it is crude and quite bulky, I'm hoping a professional will be able to help me out with a more professional solution. I am happy to add furhter clarification if required.
