Create Fillable PDF Weekly Checklist

I have a weekly checklist designed in Microsoft Word that now needs to become a polished, fill-able PDF. The finished form should: • Mirror the current layout exactly while replacing each tick box with real PDF checkboxes and adding intuitive drop-down menus where noted. • Include a drawn-signature field so the user can sign on screen from a desktop or tablet. • Contain a “Submit” button that emails the completed form to my nominated address and simultaneously saves a copy for the user (a simple mailto workflow is fine). You’ll receive the original Word file at the outset and I’ll review one quick draft before you finalise the interactive elements. The project is complete when the PDF opens cleanly in Adobe Acrobat Reader, all checkboxes toggle correctly, every drop-down shows its options, the signature can be drawn and saved, and the submission workflow delivers the PDF to my inbox without breaking the form data.
