I need a single Google Form that lets my customers check-in their equipment when they drop it off and then check it back out at pickup. The form must capture these details every time: • Customer Name • Equipment Description • Date of Check-In • Date of Check-Out • Phone Number • Payment Method All submissions should flow automatically into a linked Google Sheet where each field lands in its own column.
