Nursing Home 5-Star Tracker (Resident Data Only - No Calculations Needed)

I need a polished, enterprise-grade tool that lets our nursing home leadership monitor every quality measure that feeds directly into CMS’s 5-Star rating system. The emphasis is on the Quality of Resident Care domain. Please note this is only to track and audit the residents triggering on the MDS 3.0 Resident-Level Quality Measure Report. There will not need to be any calculations. Please read the attached directions before you start; they outline the exact data fields, naming conventions and reporting cadence we already follow. In short, I’m looking for a single pane of glass that: • Imports raw MDS data (CSV or Excel) • Generates a one-page PDF summary suitable for board meetings Accuracy must match CMS technical specifications so we can audit results against the public Care Compare site with zero discrepancies. When you hand it over, I’ll need: 1. The fully functional file/app with unlocked formulas or documented code 2. A brief user guide showing how to refresh data and regenerate reports More directions are attached which include the measures to track and what will be tracked with each measure and design ideas. If you have prior experience building 5-Star dashboards, mention it; that background will help us hit the ground running.
