Hi there, I have looked at your three forms, the outpatient consent, initial client intake and receipt acknowledgement, and I can create 3 excellent PDF documents for you one for each of these. Firstly I will convert the documents into PDF, then I will create editable sections within the PDF to allow the client to input the information you require. It's not possible in a PDF to have buttons that submit via email - but I can ensure there are clear instructions on the PDF document to explain that they are to e-mail the completed form to you.
