VBS GUI with config file

Hi,
i have a vbs script with 30 variables. I need for it a GUI with 30 input fileds it should look like

this 30 variables need to be in a config file.
it should look like.
without a config file the gui should show two buttons "save to config file" and "save and run"
on each input box a check box should be there with "hide" as title + one check box only markable when every checkbox is marked with title "Automatic"
when a config file is there only one button "Do it" should be there. The Input fields "marked" from the checkbox field schould be hidden.
a way how the vbs scribt read out all the 30 fields and use it as variables.
