Add some new functionality to our iOS and Android apps

We have an existing app (iOS and Android) that we need to add some new features -- two new screens for account creation, and account setup.
Simple project involving UI/UX design of the two screens for iPhone, iPad, and Android (small, medium, large screens), and integrating the new views into the existing app view controller flow.
I estimate this should be two to three days work for an experienced mobile app developer to familiarize themselves with our codebase, design the UI for each platform and display size, and to integrate and test the new view flow within the app.
Attached are screenshots of the two forms from our web app that need to be adapted for the mobile apps.
