Develop an appointment iphone app /ongoing work relationship

This project will be for an ongoing relationship, the finished application will belong to me as will intellectual rights etc, usual non disclosure contract. Outstanding communication skills, as well as excellent coding and design are essential. The on-going revenue in personalizing this app for individuals will be a good continued income stream, I need to be sure that we will be able to develop a strong on-going working relationship. You will need to prove experience getting the app accepted by apple on the app store, it will be a free to download application. After the initial development, their will be around 100 personalized versions a year, maybe more, I need to be sure you can handle this work load. These will all need to be successfully submitted to the app store individually. Please show The first part of the this project is to develop a very stable easy to use template app, and a demo dentist practice (I can supply the artwork or you can develop your own). Once we have this developed this correctly I will sell the system to individual dentists and you will have a contract to personalize the ongoing apps to individual dental practices. I need 2 prices, the first to develop the bones of the app and to deliver an outstanding demonstration. The second is to customize the app for individual dental practices as I sell it to them. Including pictures of key staff, address etc etc. This should be in the region of 100 per year. Brief. Design a stable skeleton app that allows the user to: 1: Home Page. See brief details of their dental practice. a: Name, Address, Telephone, email. They will be able to call the practice by pressing on the phone number, email by pressing the mail button, SMS by pressing the SMS button. 2: Who we are. A simple introduction page with a few photos of key staff their qualifications and the skills they offer. 3: Services we offer. This page will briefly describe different premium services offered by the practice along with the option to book a specific service by pressing that option. 3.1: Tooth Whitening 3.2: Veneers 3.3: Dental Spa 3.4: Crown & Bridge 3.5: 10 Years Younger Package 3.6: Mouth MOT 4: Make an appointment. This page will allow the customer to book an appointment, fields will be: 4.1: Name 4.2: Postcode 4.3: Cell number 4.4: email 4.5: Service 4.6: Send 5: 4: Cancel an appointment.
