Need simple form app to collect data to export pdf

I need an Android app which basically acts like an offline google form to collect data and photo from on site vehicle and then compress the images and data in a specific format and export as pdf to be shared in different application.
I can host the application on my own as I already have a play store account. I'll provide you with company logo for branding within application.
