Application for archive and restore information including scanned documents

Hi I am looking for professional developer who can provide the following The project is about storing information including scanned documents and restoring or updating current information using simple efficient functions and interface the interface would be in Arabic in RTL and the translation has been added. There will be login page as below: A normal login page with password reset functionality (The password will be set by administrator for users) The password which has been assigned for users expires After login the admin page will appear which will list all patients or names which has been added earlier as below: The user area above will list: 1- Number: which will display the orders of all names or information for specific person which has been added. 2- Name: Full name of the patient. 3- Old: How old is the person. 4- City 5- Region 6- State 7- Last visit of the patient 8- There will be search feature on top 9- Logout 10-Add new patient data 11-Next and Previous 12-Ascending and descending order can be done on the list based on all elements mentioned in the table. Add new patient as below: Add new patient above will do: 1- Insert the name of patient. 2- Old 3- City 4- Region which would be list the region automatically based on City. 5- State or status of the patient. 6- Notes 7- Scan the document which will result in displaying scanned thumbnails of the document beside the scan button, another scan can take place until the user click on Add. 8-Thumbnails could be displayed as slide show and print can be done. multiple scanned documents can be selected to be printed. 9-Back going back to user area Searching patients will display: Searching will be based on name by default or advance search using some filters such as specific date, old , city, region, name once clicked done the above will get displayed: 1- Name, Old, City, Region 2- Stats or status of all visits for specific person starting from last visit as time-line where status, notes , and scanned documents get displayed in ascending or descending order. 3- The details can be updated such us the correcting the name, city, old, notes, status, documents to be scanned for missing the scanning part for specific user. 4- The user can add new patient status as above by adding notes status and scan new documents which will be stored into patient record. 5- The previous scanned documents can get displayed as slideshow, printed or combined as pdf for single status. The project would be based on offline protected version ( maybe .exe) based on a computer that is not connected to the internet, the password for users will expired after 1 year, which will require the admin to set a new password using admin credentials to get it work for another 1 year.
