Client Consultation Framework - build a secure web form with conditional logic that can output to a custom spreadsheet, then to a MS SQL Server DB

This project aims to build a proof-of-concept secure web site consisting of a form with conditional logic (e.g. if they answer yes in Q2, move to Q3, otherwise move to Q4) that can: 1. Phase 1: Produce an output into a summary spreadsheet which is easy for a user to ascertain important information 2. Phase 2: Output directly into a secure MS SQL Server to create a client record and populate the relevant fields. For this project, we are happy to use any secure cloud service (preferably located in Australia) to host the site and the output data, cost permitting, but this is optional for Phase 1. We will need to move to the above for Phase 2 and figure out how to interface with an instance of a MS SQL Server 2012 hosted by a Sydney, Australia datacentre.
