SCOPE: I’m looking to add a new form to an existing website. The website is development on PHP/AJAX/JavaScript/MySQL. You will have access to a test site for development. The project is to add company profiles . The development should allow for office hierarchy, such as head office and branch offices. Companies should also be able to link or include staff members. The form will store the following information: `id` int(10) `parent_id` int(10) `status` enum('1', '0') `company_name` varchar(100) `FIELD` varchar(255) `office_name` varchar(100) `address` text `phone` varchar(255) `fax` varchar(255) `email` varchar(100) `FIELD` varchar(100) `FIELD` varchar(100) `FIELD` varchar(100) `FIELD` varchar(100) `FIELD` varchar(100) `FIELD` longblob `FIELD` varchar(100) `FIELD` text `FIELD` text `FIELD` text `FIELD` varchar(100) `FIELD` varchar(100) `FIELD` text `FIELD` int(10) All fields have already been loaded to the database. NOTE: Developer should have knowledge of PHP/AJAX/ JavaScript /MySQL/CSS. During the selection process, I will provide full requirements to qualified developers. This will allow you to fully understand the scope and determine if there are any issues. DESIGN CONCEPT: Follow existing design template. HOSTING: Hosting is on [login to view URL] PAYMENT STRUCTURE: Money will be Milestone and release upon the following: 30% - upon completion and approval of development 70% - upon successful final UAT and final delivery of all source codes and files This payment structure is a pay as you go method and will reduce risk for the developer and ensure progress for myself. COMMUNICATION: I require someone who will actively communicate with me and update me throughout the project. TERMINATION: This project will be terminated if I feel the development and/or communication process has stalled. If there is an issue, identify it and let me know. I need to turn in a status report as well. OWNERSHIP: Upon full payment, full worldwide ownership of the software/application will transfer to me.
