Build a form for simple input and Dashboard view of simple information

Create the picture below in an MS Access Report.
The information in the Report View should be based on the information collected in the form.
The Dashboard (View) will be based on a date.
The Form entry will be based on a date. The form will collect all of the numbers and items needed for the Dashboard (view).
The Dashboard will then present the information like the picture.
For example in the "Ticket Management" area there are 4 numbers needed
MyRequest - 12 CAR/JML - 18 Helpdesk - 8 Current HelpMe IT - 2
The report should create the Bar Graph like the picture and include the number at the top of it's bar.
For the "Site Application Health" the form should have a selection for which happy face to use (Red/Yellow/Green) for each department, then a spot to list 5 issues under each one.
The "Infrastructure health" area of the form should have an area to list the Project and Repairs, and put the associated date or info next to it. (and select the color for that line)
The Dashboard/View doesn't have to be exact, but it should close to the picture. Bonus for quality design work!
I've added a Mock Up of the Form that would be needed to drive that view/dashboard. (and of course that form information would need to be stored in a simple table or two. (with the Date being the unique identifier)
Further simplified description: (I realized that the term Dashboard may not be correct here)
Create a form in MS Access (Like mock up)
Have that form data stored in MS Access Table(s)
Then Generate Report (to look like picture)
based on date field
