Build a web application

A website with forms, and registered user areas, with basic report functionality on the web forms. For both PC and Mobile Phones. No need for SEO or PAGE RANKING, this is being reposted as the previous advertised project responses where focused on PAGE RANK and SEO for this site it is not required. The site is as a proof of concept as follows [login to view URL] using CSV and minimal data validation on the forms, full code to data can be provided. Course Evaluations: No login required allows users to complete form data to evaluate course delivered. Evaluations are scheduled/created through Admin Portal. All forms are identical, the variables are Instructor, Event title, Organisation and Date. Admin need to be able to pull these out as reports based on the individual who completed and by event. Ideally at midnight on the event due date the completed evaluations would be emailed in to our training email address. Pre course assessments to measure skill level, post class assessments to evaluate event delivery.
