It is more simple compared to last time: - no admin system needed - no email confirmation needed - no conditional form field display. Last time it had to wait for them to press next before showing next question. This time all questions can show at once. They are still mandatory though For the next project. It is like the online permission system but less complex: https://www.freelancer.com/projects/php/Online-Permission-Form-Basic-Admin/#/details So it is the same concept - you could probably copy it and make changes. User logs on to our portal. The permission system sits in iframe. The contacts firstname, surname, username are passed to the iframe. The form checks existing table to find related children. (See attachment for full details about the form) (The link to the old project has the details about the users and related children tables)
