Hi. Do I understand the logic correctly: The 12 questions correspond to the 12 categories (Kaizen, Vision, ...) shown in the image? Each of the answers corresponds to which color to show? This can be implemented using HTML, JavaScript and CSS (to handle displaying the circles). Currently not sure what the best way to integrate this into wordpress will be. I could probably wrap it into a plugin. Alternatively, I can provide you with the raw files (1 css, 1 javascript, and a short HTML snippet) that render everything.
