Client Record form and Diary in Web App

I am looking for a web developer with expereience in building database driven web pages, this might be PHP and mysql. I am looking for a solution to replace the paper diary which will have a lookup on entry to the client table in the DB and also display notes if available at time of booking the appointment.
The record form is three pages on A4 paper, mostly text and boolean Yes/No fields. The last page is just notes which are shown with a timestamp and username with each for audit. The other part is a diagram of the bottom of feet, the user needs to be ablle to annotate these with lines and circles and make notes to what these are and all be saved witht he client record.
The other part is a way for a user to acknowlege and consent especially for GDPR by ticking a box for the consent and if possible a way to upload some images onto the odd clients record if there is ongoing foot treatment they need to record over time.
Roles are needed, most likely user, power user and admin
I have attached a photo of their paper diary as this is what the diary webpage needs to look like, also attached is what the record card forms look like, you will see the feed diagrams that we need annotations for. Page 3 would just be a blank page on paper for the notes.
Will require a user login page also with cookie options so use doesnt have to sign in all the time on the same device.
