Form generation in Joomla

We use Joomla in the latest version with the template Helix Ultimate and SPPagebuilder from Joomshaper, as well as the component K2.
What we need is a form in the backend where we can collect data. These data come from a form that we receive from clients to publish the data on the website.
It should be a mask, some of which have selection boxes and the other free text fields.
It is up to you whether this mask is created in K2 or SPPageBuilder.

It must be a solution at the backend, where we can publish the information from the clinics. But we don´t want each time write the questions, we only want to write the answers direct behind the questions and therefor we need a mask or from, correrspondending with the Requirements (Helix, SPPageBuilder and/or K2)
