I need somebody to create a simple bootstrap-based form which collects some information: - First Name - Last Name - Email Address - Phone Number This form needs to be built as a WordPress template page and includes the header and the footer as a normal template page would. After the user submits the form, these things need to happen: 1. A new WordPress user is created using the WordPress API on my server 2. Wordpress should send the new user a notification saying that they can set their password. (Should be the default system email through WordPress) 3. The employee details are entered into a MySQL database table (Use the table provided in my google drive for this project), and the table field wp_id should be the WordPress user ID created in step 1. 4. The page should show a success message which says “Success! Please confirm your email address by clicking on the link we have just sent you to set your password. Please check your spam folder if it doesn’t appear.” Payment will be released after I test the program and it works on my server.
