SQL Scritp to Import CSV file with Text qualifer

This should be very easy. I have a simple csv file. I need to import with row terminators to split the data into rows. Also, need to use a comma delimiter to split fields. However, There is a comma within one of the fileds which is surrounded by quotes. For example, "LASTNAME, FIRSTNAME". So I need to implement a text qualifier to keep this in the same column. It probably just needs a formatfile, but I don't have time to figure it out. This needs to be done with an SQL Query script that will work for MSSQL2008. Table name can be created to match these fields. Column names don't matter to me. I need it FAST (today) for whoever can take this and write something up for me quickly. Samples are attached.
