I have a working app, for your POC!
Continuous recognition, from the microphone.
2 hours @ $50/hour = $100
POC:
* A simple macOS app, with a basic UI (start/stop button, and text field)
* Request permissions (microphone, network)
* Include Speech Services SDK
* Receive speech input
* Use SDK to transcribe (continuously)
* Output text results
Microsoft provide good documentation on their Speech Services, so that’s helpful!
Sample code is a mix of Objective-C and Swift - you want Swift.
Converting Objective-C takes extra time, so we might need to think about that.
In the long term, you are right to choose Swift.
The SDK was straightforward to install, and get started.
Do you already have a subscription key?
I develop apps for iPhone/iPad and Mac, using Xcode and Swift.

I would say that my skills are expert on iOS, and mid-level on macOS.
I choose to work on projects that I find interesting.
A medical transcription app is definitely interesting! (My daughter is a doctor.)
I now have a basic working app, which could be used for further development.
