Hi There !! I checked the page and the forms themselves look fine. The problem is more likely coming from how the Smart Tabs plugin is hiding/rendering the tab content, especially once Formidable conditional fields get involved. That also explains why accessibility breaks for screen readers and keyboard navigation. I can fix this from wp-admin itself. No server access needed.
