Add one level of options to HTML list

Hi,

I am looking to have one additional option selection page added to my website. I will add the pictures my self and a template is already in place.

The section can be viewed after you scroll down the homepage and click on "Repair" Which brings you to a list of options "Apple" "Android" "Other" "Laptop"

When you click on any of these options it brings you to a list asking about color of device.

What I am looking for is an additional set of options in between the device manufacturer selection, and the color selection.

For example:

If you click "Repair">"Apple">

I would like to have the options "iPhone" "iPod" "iPad" "Other"

If you click "Repair">"Samsung"

I would like to have the options "Galaxy" "Note" "Tablets" "Other"

If you click "Repair">"Other"

I would like to have the options "Sony" "LG" "HTC" "Other"

If you click "Repair">"Laptops"

I would like to have the options "Apple" "HP" "Sony" "Other"

These options MUST show up in the confirmation email that I recieve once a client goes through the selection process.

Easy enough? Hoping to have someone complete this within an hour, if you know what you are doing it is a quick process.

I will add pictures later myself, a dummy picture can be used for the time being.

I need to have the header and footer replaced on a specific page: www.seatoskycellrepair.com/iphone

i want to have my standard header and footer on this page, and not the template one from apple that is on the /iphone page now
