Custom Magento page needed

We have a Magento based ecommerce site called [login to view URL] and we are looking to have a custom page created. We are looking to add roll labels to our website in a similar format as [login to view URL]

We do not need the "shipping" section, starburst shape, or white vinyl material options. We will need to be able to easy change prices and options as well.

If it helps we have the Mico Custom Math Pricing extension installed on our Magento site. The extension can be seen here [login to view URL]

We need this page to function perfectly and if you can complete this job we will be having multiple other projects immediately after.

This custom page must be a product that will incorporate with our shopping cart and checkout.

We want the item page to be incorporated into our existing product page theme. We do not need anything other than the options to function this way. Please keep this in mind during the quoting process. You can see an example of one of our product page layouts in the link below.
