I need an online registration form that can be used over and over again by 17 different sales reps to register potential clients for a free lunch that we bring to their office where we collect a email address, their position in the firm and their membership number.
I will supply all artwork and layout.
Step 1
User goest to the website
Selects from Drop Down menu 1 of the 15 DM’s in this user case Tom Nordhof
Step 2
The main site appears and Tom enters the Firm Name, Address, Zip and enters one contact name and number of the office manager.
They then enter the Date and Time of the Lunch
They add the name of the Restaurant that will be catering.
Once all fields a are filled out they will hit a create Lunch and Learn button.
It creates the website page mock up and it tells them to proof the info then hit the create button.
An email link is then created and sent to Tom's email address.
This is now an active link and can be sent to the main office contact to fill out themselves or pass around the office. It is like a real time form.
The site explains what they are signing up for based on the info provided above.
The first field will be to enter their name and hit confirm that they would like to attend.
If they cannot attend there will be a link they can click to send them to the wondoor online course.
If they confirm their name they will advance to a the next form that asks if they would like to be on the list for the free lunch.
A new field comes up asking if they would like to receive an membership credit for this course and if so to enter their AIA number and email address so that their certificate can be sent after attending the meting.
They would hit submit and their info would now appear at the top of the updated virtual list.
This list will show up to anyone accessing the link. There should be a "not able to attend button" that they can check next to their name which will leave them on the list but put a light strike through.
1 week before the L&L an email reminder will be sent to the DM, MA and main contact. Showing the the people who have registered with a total count a the bottom of the list.
1 Day before a final email will be sent too the DM, MA, contact and all other names on the list a reminder of their such appointment and the list of those registered again having the opportunity to opt out or add another name if needed. The DM can then print up the sheet and take it in for orders and for using as a sign in sheet where it can be based around and attendees can initial their name.
1 hour after the meeting a summary of the meeting registrants will be sent to the MA and and Manager. This list can be updated if a new person came in last minute or de-selected if they had a no show.
They would hit submit and this info would be entered into the system. And a final report sent to the administrator.
