Create my Document

I need a website built to sell my Document. You must be familiar with building a site along with how to merge Data into a form to create a Document. The website will need to have approximately five questions I furnish for you to be asked on the site and answered by my client and based on the answers given One of three Documents must be chosen to be the one to use automatically. I will furnish you with the questions needed to be asked and let you know which form to use based on my client’s answers. There are approximately 25 Entries that will be made by my clients, about half of which should use drop down menus for the correct dates etc. I will need the drop down menus added by you. The Data must then be merged into the correct form to create the perfect document for my client. I have all three documents prepared in PDF along with the data fields in place to show what information goes in that area. I can also furnish an example of all three possible finished documents. Each form will use the same data entries to create the document. I will two means of payment (PayPal and or Credit Cards) before my clients may receive their document upload or emailed to the client automatically, they should also have the option of having their document mailed to them for a fee. It must be ios(iPhone) and android friendly. I will need two admins. I will need to be able to easily make changes when required.

There are four possible forms that could be the one possibly used to create the final document, not three.
