Convert PDF forms to HTML forms(repost)

Our client needs the following 11 real estate forms converted from PDF to HTML. Most forms have multiple pages. We just need the forms layouts designed we will take care of submission to the database. These forms will then be incorporated into our application for collecting data into their MS SQL database.

The HTML forms need to be as close as possible to their PDF counter parts. Each page pdf a form should be a separate HTML page. For, example if the PDF form has 6 pages then the conversion should result in 6 HTML pages for that form.

We would like to keep the resulting HTML pages size as small as possible. If we can keep the .html pages below 100k in size that would be ideal. The results should be compatible across browsers -- IE, Firefox, Safari. If possible try to avoid using absolute position in the html pages.

The deadline for this project is very aggressive, so I will prefer bids that can provide guarantees of fast delivery.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

We will NOT consider any bid that does not include a sample html form. You can post a sample of a previous project that has a complex html form or convert a small section of one of the PDF that I have attached. This is a critical project and if you can not complete it in seven days and demonstrate the ability to deliver do not waste your time. We will also select 2 bidders to ensure the project success and pay both coders accordingly. Our company also typically pays bonuses for exceptional work.

## Platform

The results should be just HTML no business logic and compatible across major browsers -- IE 6 or better, Firefox, and Safari.

If possible try to avoid using absolute position in the html pages.

You can use ASP.Net server controls or standard HTML controls although ASP.Net controls are preferred as we will be incorporating these forms into our ASP.NET web application.

You can use CSS or tables to produce the desired results for the form layout.

I have included a sample of a converted file which demonstrates exactly what we are looking for.
