I need to make an identical survey style. It should do a popup on the first page just like that and have that background music playing. Once you finish the survey it shows you results of products. I need to have an admin panel for this where I can login and change the products each time. I should be able to upload the photo, name, regular price, your price, shipping and quantity left for each product. It should display up to 4 products. The most important thing is that this survey needs to pass referral data of the website that was targeted. In the URL you see ?keyword=URL this will be passed with a variable {keyword}.
