Create form from PSD, Integrate form with payment

We have a form that needs to be created from our PSD file. It is just name and email forms, and then an area that needs to be clicked to forward user on to payment. I uploaded a PNG file that shows the name and email form to this project. We currently use [login to view URL] for our payment management after they click add to cart area.
So the email will have to be sent to [login to view URL] once the user clicks "add to cart" in the clickable area, then the user will be forwarded on as they would normally with [login to view URL] script for payment. It's very simple, you just have to build the script that sends the email to [login to view URL], and place it into the digitalaccesspass process.
