I need a Date of Birth form created for the splash page of a website that sells alcohol.
Ideally I would like:

2) automatic detection of the user’s location using Google Analytics, IP trackers, etc.
3) set up to install a cookie so the visitor doesn't have to resubmit DOB every visit
