Design, CSS and PHP Expert required

We need an expert in Design / CSS/ HTML and PHP to do the following for us. All these tasks are simple and small tasks.
1. Email to subscribers to join Facebook Fan Page - design idea will be provided - where possible images also.
2. Change Header - currently we have only subscribe button. We would like to add First Name and Last Name field in the header and subscribe button - linked to MailChimp code - code will be provided to be modified by the provider
3. Design pop up box for promotion and add subscribe fields - with Mail Chimp code - site 1
4. Wordpress Blog already install - to copy content from old to new database as we need to fix the links
PLEASE NOTE WE NEED EXPERTS - so please do not waste time if you are not experienced. We need this to be completed in next 24 hrs.
