I need to have someone create a web enabled PDF form that will allow someone to fill in fields in a PDF form they access from our website and submit their data to our database. The project includes coding the PDF form appropriately as well as building the database to reside on a win2003 server. It should have all the standard protections of doing web data collection. We want to stay with PDF as the collection front end because there format and consistency issues that need to be maintained.
We will need basic data import and export functions as the final data will be analyzed in excel.

This includes some basic install on our servers. I think that ms access can handle this task.
