Convert a MS Word form into a Adobe PDF Form

I want you to convert this MS Word form to PDF Adobe LiveCycle Form.

The important thing is that layout match exactly as the original. This MS Forms has a passwords security. I need you to break it if necesary for the convertion and convert using Adobe LiveCycle designer

NOT NOT TRY TO DIRECT REPRODUCE THE FORM BY HAND!!

MY INTEREST IS TO IDENTIFY PROPER USE OF ADOBE LIVECYLE OR ANY OTHER SOFTWARE TO AUTOMATE THE PROCESS TO A FORM!!!

I have about 300 additional forms and this is the test trial to make a businss relation with a coder that efficientlyconvert the remaining MS WORD forms.

## Deliverables

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
