457560 convert data

we have several .txt files (3) that are in mailing label format that need to be converted to .csv format. See attached file. We need the file to be converted as:

name,address,city,state,zip

For example the file attached needs to be converted as

Juan C. Alvarado,933 N 10th St.,Reading,PA,19604

Alex Aviles,226 s 4th,Reading,PA19602

etc.
