PHP Custom modification and coding

Hi,

I basically require a PHP programmer to assist me modify my already existing system. I need a few things done:

1) I currently have a form that submits to database. I need you to just rectify the calendar issue where it overlaps the text. I also need you to space out the fields to make it more presentable.

2) I need a page done where there is a login. 4 users. Each user login and will be able to submit details into teh database or make some modification. I have the page done up in HTML. I also will advice you on the tables the data is stored.

3) Based on the submission above i need to view a report a simple report.

4) Finally i already have a script done, to display some information but its on a blank page without my company header etc. I need you include my company header.

I need you work with me on this today.
