Project is pretty straight forward. Need two forms built within an existing website template.

I need a custom quote form created. Requirements:
1) it must keep within the same style/theme of the template. no exceptions. (the sample image I used is NOT to be recreated style wise... only use as sample for which fields need to be added
2) it must maintain responsiveness (bootstrap 3.3.7)
3) it needs to have a confirmation page created that will show up once user submits form
4) it should send information captured in form to two different email addresses.
In addition to quote form:
I need email captchas added to
1) quote form being built in this project should include captcha and confirmation page once user submits information.
2) existing contact form - this page is already built and just needs captcha added as well as confirmation message once user submits information.
3) email field submission in footer of website.
In the footer section of website, there is an email submission field. I need a captcha added to it. Once user enters email address, captcha should pop up to prevent spam. Any captcha is okay.
